Our 24-hour skilled nursing care facilities provide treatment, care, and rehabilitation therapy for patients after hospitalization for joint replacement or other orthopedic surgery, traumatic injuries, brain injuries, strokes, open-heart surgery, or a serious illness.

 
Our services include:
  • Daily physical, occupational and speech therapy
  • IV Therapy
  • Wound Care Management
  • Orthopedic Care
  • Specialty care for patients recovering from Stroke
  • Hospice Care
  • Pain Management
  • Post Surgical Rehabilitation
  • Amputee Rehabilitation
  • Renal Dialysis on-site
  • VitalStim, for dysphasia, on site


For patients who need complex medical treatments, we provide a distinct unit with:

  • Intravenous and internal therapy
  • Intravenous antibiotic therapy
  • Respiratory services / Tracheostomy care and services
  • Pain management
  • Post-surgical stabilization
  • Specialized skin and wound care

Our sub-acute program provides physician-directed care, social services and discharge planning and counseling, nutritional counseling, therapeutic recreational activities and individual counseling and support.

The External Case Management staff also makes daily on-site visits to the following hospitals:

  • Christ Hospital, Jersey City
  • Jersey City Medical Center, Jersey City
  • Greenville Hospital, Jersey City
  • St. Mary Hospital, Hoboken
  • Bayonne Hospital
  • St. Francis Acute Rehab Center
  • And other NJ hospitals
